At APluscare, our service and support plans are based on person-centered thinking. Teams work with individuals and those important to them to develop individualized plans. Our process starts with identifying how a person wants to live and balances that with any health and safety issues. We provide care that fits your specific needs, no matter how small or extensive.
APluscare utilizes the Developmental Disabilities Resource Tool (DDRT) to identify abilities in areas of cognition, communication, self-care and mobility to help develop the support plans that will enable individuals to successfully live as independently as possible. Based on each person's needs and/or assessments, we maximize the impact of all available resources (local, state and federal level) through precise budget planning, and maintain effective management of these resources through proper accountability and monitoring of usage. We constantly seek and match resources with the right person based on the needs of individuals in our care.
APluscare makes a point to stay up-to-date with research results and staff training resources and requirements from New Jersey’s Department of Human Services (DHS), Division of Developmental Disabilities (DDD), and Department of Children and Families (DCF); Rutgers’ Boggs Center; and other organizations that assist people with developmental disabilities.
