BEHAVIORAL HEALTH SERVICES
APluscare strives to maximize each individual's choices and independence, tailoring all supports and services to fit their unique profile. Individuals in our care are consistently empowered to communicate their choices in all areas of services provided, and are encouraged to participate in community self-advocacy groups that help them understand their rights and options.
Specific services and provisions we provide include:
Individualized Residential supports and services for children and adults with developmental disabilities
Support Services are provided to each individual in their choice of home. We offer individuals a wide range of services and creative options for housing that promote their self-sufficiency and independence to the highest degree possible.
Flexible Supports
APluscare provides support based on the schedule and needs of individuals, not programs. Individuals receive support where, when, how and with whom it is needed. Supports are flexible enough to be adjusted according to the individual's changing needs, preferences and desires.
Natural Supports, learning and technology
Whenever possible, APluscare provides supports which are natural to the place, time, delivery and person. The design of individual supports seeks to create opportunities for individuals to learn to provide their own support, for it to be more natural, and to use technology that may give them more control.
Supportive/Self-Directed Housing Services
This type of service focuses on what people can do, provides support for things people cannot do, and provides opportunities for them to learn how to do the things they want to do.
Inclusive Language
Our services include language that is natural and promotes inclusion. Thus, places people live are described as Joe's home or Mary's home; people clean their home and do their laundry rather than learn programs; people live with roommates, friends or family members, not with staff or providers; friends come over to visit, not volunteers.
Financial and Community Resources
Utilizing various resource guides for creative housing for those with developmental disabilities, we offer solutions that meet the financial needs, as well as the housing needs, of individuals in our care. We also provide assistance with obtaining applicable resources at the local, state and federal level.
Stand Alone Behavioral Support (SABS)
SABS are time-limited direct services provided to restore an individual's mental health and/or behavior to a pre-crisis baseline level while allowing the person to remain in his or her current living arrangement and/or employment/day support. Supports may include one-on-one supervision, redirection personal controls, and implementation of a formal behavior support plan. Our well qualified professionals will determine the type, intensity and duration of behavioral supports needed in accordance with the Crisis Plan outlined by the Emergency Interdisciplinary Team (IDT); prepare formal behavioral plans; and provide staff training as needed.
