day habilitation program coordinator

Salary, Full Time

Responsibilities

  • Coordinates all necessary team meetings for the program
  • Develops, modifies, and/or recommends necessary amendments of the service plans to Support Coordinators/Case Managers
  • Demonstrates key knowledge of Division of Developmental Disabilities regulations, circulars, and licensing standards
  • Maintains service programs compliance with DDD, DHS, and other relevant state departments as well as federal rules and regulations
  • Maintains service program's full adherence to the Policies and Procedures of APluscare
  • Consistently monitors the well-being of individuals served
  • Develops, distributes as appropriate, and monitors staff schedule
  • Coordinates the planning and implementation of care and support plans, and assumes the responsibility for the implementation and appropriate documentation as needed
  • All other duties as requested or assigned by APluscare

Requirements

  • A bachelor's degree in Nursing, Health Care Administration or related
  • A minimum of 3 years of experience working with individuals with developmental disabilities is preferred
  • Employee must cooperate with the licensee and department staff in any inspection or investigation
  • Employee must successfully complete and demonstrate proficiency in all areas of required training
  • Push, pull, and lift up to 50lbs
  • Valid Driver's License
